Check In/Required Forms

CHECK IN INFORMATION 2025

Teams are required to submit Check-In materials electronically prior to Friday, August 22, 2025 via gotsport account.   

If teams do not have required forms your game is subject to forfeiture if you miss registration and cannot comply with requirements by Saturday morning.

Please submit the following materials online for check-in:

1. Upload a copy of your team’s certified roster – REQUIRED.
2. Player passes with photo required for all participants – REQUIRED.  Player passes will be cross-checked against the certified roster and waiver forms.  Player passes should be retained during the tournament by the team representative and must be made available to the referees at each game.  All player passes must be from the same governing body (USYS or US Club Soccer).
3. Combined Liability/Medical Release – REQUIRED signed by an adult for each participant.

Once form is open, click to bring to pdf, print, obtain signatures, upload to Gotsport account.

4. If using guest players, a guest player form, uploaded along with copy of their photo player pass to the gotsport account. Signature authorizations should also appear on the waivers referenced in #3 above. The guest player form request can be found at: Registration – Ohio Soccer Association (ohio-soccer.org). Once completed by OSA, upload form to Gotsport registration page.

5. USYSA teams from outside Ohio must have proof of permission to travel.  Please obtain this from your state’s soccer association and upload to Gotsport.
